Porokeratoses have a raised keratotic ridge with a central groove that, yeah, could look like a big wall surrounding the lesion itself (hence, the description). There is an increased risk of malignant transformation, so if patients have these, I tell them to keep an eye on them and let us know if they notice any changes, or if the area seems to bleed easily. People who have many of them on their arms and legs have a condition called DSAP - Disseminated Superficial Actinic Porokeratosis, where the malignant transformation is low but still exists.
