Porokeratoses are actually inherited, autosomal dominant skin conditions. In other words, they tend to run in families. They have a distinctive skin finding that we learn as dermatologists in our residency training and we called it the "Great Wall of China" finding... Hey, at least we don't compare it to food like we normally do!
Porokeratoses have a raised keratotic ridge with a central groove that, yeah, could look like a big wall surrounding the lesion itself (hence, the description). There is an increased risk of malignant transformation, so if patients have these, I tell them to keep an eye on them and let us know if they notice any changes, or if the area seems to bleed easily. People who have many of them on their arms and legs have a condition called DSAP - Disseminated Superficial Actinic Porokeratosis, where the malignant transformation is low but still exists.
